My only question for this and any other complication of Diabetes is "at what level of glucose control does this come into play.  It seems that all health professionals consider anyone with Diabetes is going to suffer from the same complications.  There is never a distinction of at what level of control or what A1c this is occurring. 
Type 2 since 1993.  Control with Metformin and LCHF diet.  76 YY
